Toronto Rehab - Advancing Rehabilitation, Enhancing Quality of Life

Seniors in our two inpatient programs stay on average six weeks. In that time, they will see a number of rehabilitation specialists to develop individual care plans to respond to any number of physical, mental, emotional and social problems. Plans are regularly reviewed with family members, who are encouraged to participate fully in the rehabilitation process, in order to learn how to care for and support the individual at home. Our team can also provide family members with education and support group sessions if needed.

Geriatric Rehabilitation Service

While the medical diagnoses for admission into the geriatric rehabilitation inpatient unit are multiple and varied, many patients need help to cope with the physical, mental, emotional and social problems associated with aging.

Common goals include improving mobility and balance, fulfilling one's daily living activities, and feeling safe and secure while living at home. Toronto Rehab's team of specialists treats therapy as practice for regaining one's independence, and many patients end up going home, while others require the additional assistance provided in a retirement or long-term care facility.

Toronto Rehab also offers the only rehabilitation program in Ontario that is able to provide a regimen of daily dialysis and daily rehab to seniors.

Dialysis

Toronto Rehab offers on-site dialysis to seniors who require hemodialysis and specialized rehabilitation. To make this possible, Toronto Rehab, with its partner University Health Network, provide dialysis two hours a day, six days a week instead of the more common four hours a day, three days a week. This important difference leaves patients with more energy and the ability to participate in daily therapy.

Geriatric Psychiatry Service

This inpatient unit provides treatment to seniors who are experiencing behavioural problems, such as wandering, aggression, and agitation, related to Alzheimer disease and other dementias.

Treatment often involves a combination of medication, assessing and monitoring behaviour, group programs, and individual and family counselling.

Most patients stay about six to eight weeks before they return home, or to a facility that can provide them with the appropriate level of care.

Geriatric Day Hospital

The day hospital accepts seniors from the community, or patients who have completed our inpatient program and require ongoing therapy. Therapy is provided on an outpatient basis twice a week for three to four hours a day, usually for three months. Seniors can practice their mobility, balance, fine motor control, and other skills needed to live independently.

Upon discharge home, patients may be referred to other Toronto Rehab services or to a Community Care Access Centre for ongoing care and support.

We also offer two specialized medical clinics for seniors. Assessments include an examination of the person's physical, mental and emotional health, current medications, and support systems. Individuals are often referred to other Toronto Rehab or community-based services for specialized help following the assessment.

The Geriatric Medicine Clinic assesses seniors who are experiencing:

Memory loss
Multiple physical disabilities
Mobility problems, falls
Bowel or bladder difficulties
Challenges coping with multiple chronic illnesses or multiple medications
Inadequate support at home
Possible abuse or neglect, alcoholism, poor nutrition or weight loss

The Geriatric Psychiatry Clinic provides specialized psychiatric assessments of seniors who are experiencing:

Depression
Late onset schizophrenia
Paranoia, delusions or hallucinations
Anxiety disorders
Behavioural disturbances associated with dementia
Other psychiatric disorders.

Outreach

Our outreach team assesses seniors in their homes or in long-term care facilities, living in the downtown core and west Toronto, who are experiencing deteriorating mental status, mobility problems, accessibility problems at home, incontinence or inadequate support.

Often seniors can comfortably continue to live in their homes with some physical modifications, while other times the team may recommend an inpatient assessment, formal rehabilitation program, or higher level of care.
