Brain injuries and illnesses can cause devastating effects to an individual’s memory, concentration, planning, problem solving, talking, behaviour, emotional stability and independence. Sadly, brain injuries and illnesses are prevalent in our society. Stroke affects nearly 50,000 Canadians each year. In Ontario alone, nearly 18,000 adults suffer from some form of brain injury annually. Whether the effects of a neurological injury or condition are mild or severe, rehabilitation is critical to regaining function, mobility and independence and enhancing quality of life.

Toronto Rehab’s Neuro Rehabilitation Program offers a full continuum of care, including inpatient services, outpatient services and follow-up clinics. The program has been designed to allow patients to enter at any point along the care continuum and to transition smoothly between services based on individual need.

Neuro Rehab patient

Patients are referred to the Neuro Rehab Program from acute care hospitals across Ontario, Canada and around the world, or directly from a family physician. A doctor's referral is necessary for admission. We are pleased to provide important information for referring professionals about making referrals.

The Neuro Rehab Program includes three specialized services:

  • the Brain Injury Service
  • the Stroke Service
  • the Neurology Service

Within the Brain Injury Service, care is separated into two distinct streams: one for patients whose rehab needs are primarily cognitive and one for patients whose needs are primarily physical. Special assessments, treatment and education are also offered for individuals with multiple sclerosis. An innovative Chronic Pain Service for patients looking for self-management strategies and peer support to deal with their pain more effectively is also available.

Toronto Rehab now also offers a specialized Comprehensive Spasticity Management Clinic.

Rehabilitation services are delivered by an interprofessional team of health care experts representing more than a dozen disciplines. Because the effects of neurological injuries and conditions affect people in many different ways, the Neuro Rehab Program delivers individualized treatment to meet each patient’s needs and goals. Patients’ family members are encouraged to participate in the patient’s therapy whenever possible. Patient and family education is a critical component of the rehabilitation process at Toronto Rehab.

Toronto Rehab is committed to helping patients successfully transition from hospital to the community. The hospital works in partnership with families and community care organizations before patient discharge to ensure the necessary supports are in place outside of the hospital to help patients maintain the gains they made during rehab and to continue improving on their own. The vast majority of Toronto Rehab’s Neuro Rehab inpatients return to supportive home environments after treatment. Many people continue therapy with Toronto Rehab on an outpatient basis after discharge.

All of Toronto Rehab’s patient care is informed by a robust, world-class research program. Advances in research are rapidly translated to the clinical setting so that patients can continually benefit from the most cutting-edge therapies, methods and equipment.

New transition program: LIFEspan

Toronto Rehab is currently working in partnership with Bloorview Kids Rehab to implement the LIFEspan service– Living Independently and Fully Engaged, a program that helps people with childhood-onset disabilities transition from pediatric care to adult health services using an interdisciplinary approach that mirrors the pediatric system. Toronto Rehab now offers a specialized interprofessional spasticity management clinic that provides timely access to a full range of individualized treatment options and follow-up. Through the clinic, an interprofessional team of health care professionals provides patients with leading-edge assessment, consultation and treatment.
